The WOW study has a number of questionnaires. 

The Enrolment Questionnaire is the first questionnaire to complete when you come on to the study and is to be completed when you take the third enrolment swab. 

The Three Monthly Questionnaire is to be completed routinely every three months. We will send you an SMS reminder when each questionnaire is due.

The New Partner Questionnaire is to be completed between the routine three month questionnaire periods only when you have a new partner or you develop symptoms of BV (ie: odour or discharge).
